Izzie can be resuscitated after heart failure, but George dies. The entire hospital is in mourning, and Callie and Izzie in particular are devastated. His mother Louise also comes to George's funeral. Izzie and Callie decide to donate George's organs. Derek reports to Richard that the board of directors wants to kill him and instead wants Derek to be their new boss. Derek gives Richard time for change. Cristina and Owen may by order of Dr. Wyatt can't have sex because of Owen's trauma, which isn't easy for them. Richard refuses Callie the promotion to senior physician, who then resigns.

Despite having cancer, Izzie comes back to the hospital and spends five hours in the operating room. She overworks herself and forgets her pills. Alex forces her to be disciplined. Cristina tries her hand at paediatrics so as not to be fired, although she has nothing to do with children. Arizona is upset when it realizes its strategy. Callie announces to Arizona that she wants to move in order not to be fired again by Richard after the merger. Arizona doesn't want a long-distance relationship, so they want Callie to stay. Derek and Richard have an argument about the chief medical officer. As a result of the wave of layoffs, many interns are fired, including Pierce, Claire, Dani, Leo, Laura and even the pregnant Megan. The nurse Olivia Harper is also affected.

Callie's first day as a senior physician is also the first day of the doctors from Mercy West at Seattle Grace. The interns start a bitter competition among themselves. Cristina realizes that her Dr. Jackson Avery is superior in some ways. Lexie gets involved with Dr. April Kepner to each other. Finally, Lexie April steals the notebook with the help of a trained burglar. But Lexie realizes that it was a mistake and apologizes to her. When Izzie finds out that Charles is pissing on her, she starts a bitter argument, which leads to her making a huge mistake: she prescribes the wrong dose of medication to a patient and the patient nearly dies. Because of this, Richard fires her. Izzie leaves Alex a suicide note and leaves Seattle. Meredith spends all day in bed in the infirmary.

Derek then gives Meredith the choice of whether he should tell the board of directors about Richard and reinstate Izzie himself as chief physician, or whether he should remain silent and thus save Richard from losing his post as chief physician. Meredith chooses Izzie, but since Alex doesn't want to start over with her, Izzie leaves Seattle forever. Teddy stays at Seattle Grace to fulfill her contract. Meredith discovers that she is pregnant. Cristina breaks up with Owen because Owen cannot decide between her and Teddy. Gary Clark arrives at the hospital with the intention of murdering Derek, Richard and Lexie because he blames them for the death of his wife. On the search for Derek's office he meets Reed, who rejects him angrily, whereupon he kills her with a head shot. Alex is also shot, who is being looked after by Mark and Lexie in the lounge. April finds Reed's body and tells Derek about it. The hospital is then locked. Callie and Arizona suffer from this and now have to work together in a very small space after their separation. Arizona tells Callie that she doesn't want to change her mind because she doesn't trust Callie and how quickly she falls in love. Finally, Clark also meets Miranda, a patient, and Charles. Since Miranda poses as a nurse and the patient plays dead, they escape him, but Charles is shot and taken care of by Miranda. After Clark continues to murder innocent hospital staff, the climax comes on the large gallery of the hospital. Derek is shot in front of Meredith's eyes.

Teddy and Owen make it outside with their injured man, but Owen goes back to the hospital because of Cristina. Lexie and Mark also manage to take the seriously injured Alex, who thinks Lexie is Izzie, to another hospital. Lexie confesses her love to Alex in front of Mark. Miranda is less fortunate - with the elevators no longer working, there is no other way but to let Charles die. Miranda pays his last respects. Derek has to be operated on by Cristina and Jackson as there is no senior doctor on the floor. Owen, who is also shot by Clark, is operated on by Meredith and April. Meredith miscarries before she can tell Derek about her pregnancy. When Clark gains access to the operating room in which Derek is being operated on, Jackson uses a trick to trick him into believing that Derek is dead. They finally manage to save Derek. Richard goes to the hospital to negotiate with Clark. Clark eventually kills himself, not Richard. Arizona gives in and wants to have children with Callie.
