In the seasons there were the following cast changes in the finale:
- USA - Crystal Cox has been replaced by DeeDee Trotter and Moushaumi Robinson has been replaced by Monique Hennagan.
- Russia - Natalja Nasarowa ran for Natalja Iwanowa and Natalja Antjuch for Tatjana Firowa.
- Jamaica - Novlene Williams replaced Ronetta Smith.
- United Kingdom - Helen Karagounis has been replaced by Donna Fraser.
- India - Sathi Geetha ran instead of Manjeet Kaur.
A three-way battle between the last three world championship seasons was expected. The reigning world champions from the USA met Russia - world champions 1999 - and Jamaica - world champions 2001 .
US starter DeeDee Trotter broke her season in the leading position. The Russians and Jamaicans followed closely. But on her final straight, the second US runner Monique Henderson increased the lead considerably. Russia moved in second, Jamaica in third. Poland followed some way behind in fourth place. Not much changed in the third round. Sanya Richards-Ross handed the USA over to her final runner Monique Hennagan with a clear lead, Russia moved behind just before Jamaica. All other teams were already far behind in the following places. The US season could not be deprived of victory in the final round, Russia also defended second place. The Jamaicans crossed the finish line in third place with a lead of more than three seconds over Great Britain - the British runner Lee McConnell had overtaken Grażyna Prokopek from Poland in the last few meters. The Romanian relay came in sixth place. In the first three places there was exactly the same run-in as at the world championships last year.
For the US season it was the third consecutive Olympic victory over 4 x 400 meters , the fourth victory overall.
