The 1991 San Marino Grand Prix took place on April 28 at the Autodromo Enzo e Dino Ferrari and was the third race of the 1991 Formula 1 World Championship .

As new models, the Footwork FA12 , the Brabham BT60Y , the Fondmetal GR01 and the Benetton B191 were ready in time for the start of the European season.

At AGS , Stefan Johansson was replaced by the newcomer Fabrizio Barbazza .

Michele Alboreto had an accident during training in the Tamburello curve, where one of the new Footwork GR01 was badly damaged. Both Footwork pilots failed to qualify.

Ayrton Senna reached the third pole position on the third race weekend. Next to him, Riccardo Patrese qualified for the front row ahead of Alain Prost and Nigel Mansell , Gerhard Berger and Stefano Modena .

Due to rain, there were no time improvements during the second qualifying round on Saturday.

It started raining shortly before the start. On the last of several introductory laps , Prost spun out in the Rivazza area and had to forego taking part in the race, unlike Berger, who also spun.

Patrese took the lead after the start and initially gained a lead over Senna. Nigel Mansell retired after a few meters due to a collision with Martin Brundle . For Nelson Piquet and Jean Alesi , too , the race was over after a few laps after driving errors on the wet track.

On the fourth lap, when the track was already drying, Bertrand Gachot damaged the front of his car in a spin in the Tamburello curve. Shortly afterwards, Maurício Gugelmin collided with the damaged vehicle. Both were still able to continue the race.

After ten laps Patrese had to head for the pits due to a misfire, leaving Senna in the lead. Although the Italian returned several laps behind on the track, he gave up a little later. Ayrton Senna was in front of Gerhard Berger, Stefano Modena and Roberto Moreno after Satoru Nakajima , who had previously been fourth , was eliminated due to a gearbox failure. When Modena had to give up because of a similar problem, Moreno came in third, followed by JJ Lehto , Pierluigi Martini and Eric van de Poele . In the 52nd lap, Lehto took third place from Moreno. When he retired shortly afterwards, Martini came fourth ahead of van de Poele, who, however, was ranked ninth due to a lack of fuel. The two Lotus pilots Mika Häkkinen and Julian Bailey took fifth and sixth place . They thus achieved the first ever World Championship points of their Formula 1 careers. JJ Lehto reached the podium for the first and only time.
