The same drivers who contested the San Marino Grand Prix two weeks earlier also competed in Monaco.

Alongside Ayrton Senna , who secured pole position as on each of the three previous race weekends of the year, Tyrrell driver Stefano Modena surprisingly qualified for second place on the grid. Riccardo Patrese followed next to Nelson Piquet . Nigel Mansell and Gerhard Berger formed the third row on the grid.

Martin Brundle was disqualified because he did not follow a request to weigh in.

Although Modena got off to a good start, Senna kept the lead and did not give it up until the finish. In the first corner Berger collided with Piquet. The Basilian was eliminated immediately. Berger drove to the pit to have repairs carried out on his car. When he tried to clean his visor a little later, he rammed the barrier in the area of ​​the swimming pool and was eliminated.

Pierluigi Martini behaved unsportsmanlike by preventing his compatriot Stefano Modena from lapping. He was the first driver in Formula 1 history to receive a 10-second time penalty in accordance with a newly introduced rule, which was completed in the pit lane.

On the 30th lap, Alain Prost overtook his former team-mate Nigel Mansell and thus came fourth. Another change came 13 laps later when Modena's engine exploded in the tunnel and Patrese slipped on the oil that got onto the track. So Senna was clearly ahead of Prost and Mansell, who were now second.

On lap 63, Mansell attacked Prost and overtook him as he gave in to avoid a collision. Shortly afterwards, the Frenchman had to go to the pit unplanned. He reached the goal in fifth behind the victorious Ayrton Senna as well as Nigel Mansell, Jean Alesi and Roberto Moreno . JJ Lehto , who had been sixth for several laps, did not reach the finish due to an engine failure. Instead, his team-mate Emanuele Pirro received the last point of the day.

After four victories in a row, Ayrton Senna led the world championship with 40 points in accordance with the new point rule and was 29 points ahead of second-placed Alain Prost.
