Meredith breaks up with Derek, but learns that he came to Seattle because Addison cheated on him. George has to play the hospital spy for Webber, who is bedridden after his operation. When he learns of the affair between Cristina and Burke, he is in two minds whether to tell the chief doctor about it. He decides against it. The ever popular bar owner Joe has to undergo surgery for an aneurysm in his brain, but he has no health insurance. George works with Richard to ensure that Joe is supported with funds from the hospital fund because otherwise he would be bankrupt after the operation. Richard agrees. Alex is deeply impressed by George's commitment and makes up with him. Richard appoints Preston as his deputy on the grounds that Derek does not adequately separate work and personal life. When Preston learns this, he separates from Cristina without knowing that she is pregnant. | |||||||

The relationship between Meredith and Derek is still very tense. Cristina tries to avoid Preston. Richard gets a visit from his wife Adele, who desperately wants Richard to retire. Alex takes care of a patient who could donate part of his liver to his father after a car accident, but does not know whether to do so because the father beats his mother. Alex, who has himself experienced violence in the family, is there to advise him. George talks to Olivia and tells her that he has fallen in love with another (Meredith). Although Olivia is sad, she is glad George told her the truth. A patient caused a stir in the hospital after swallowing dolls' heads. | |||||||

Meredith 's mother, Ellis, who has Alzheimer's disease, is hospitalized on suspicion of liver cancer. The tumor turns out to be benign. Ellis thinks that George, who looks after her, is her husband, Thatcher. Derek and Addison get close again. Izzie gets along better and better with Alex, but cannot understand why he is only charming towards her and is more than insensitive towards everyone else. Meredith gives Derek an ultimatum: Either he signs the divorce papers or it's over. As a sign, Derek is supposed to come to Joe's bar, but no one appears. There is a power outage in the hospital. George has to perform a heart operation in the stuck elevator and earns a lot of recognition for the successful operation. Alex, who was also present, had hesitated at the crucial moment. Because of the unsuccessful date, Alex and Izzie argue. Cristina and Preston get back together. All doctors have to go to hospital immediately because of a train accident. With the interns at Joe's bar earlier, Meredith is drunk. Miranda is also not very enthusiastic about having to interrupt her tenth wedding anniversary. As a result of the train accident, many patients are brought in. George, Meredith, Miranda, Preston, and Derek have two patients who were pierced together by a pole. There is only one chance of saving one at the expense of the other's life. After the man has less serious injuries than the woman, the doctors decide for him. Richard wants to sew a leg back on a man that was severed in the train accident. However, Richard first gets a second left foot and then a woman's foot into the operating room. Cristina tries to find the right leg to score points with Richard, but Alex beats her with the right leg and gets the laurels. Izzie's and Addison's patients are two best friends who are both heavily pregnant. As a result of the train accident, one of the babies had to be delivered in an emergency operation, but this operation ran without complications. An apparently pregnant man is admitted to the hospital. It turns out that he has a rapidly growing teratoma . This is the hospital sensation and Izzie and Cristina make a profit from it. Meredith, whose failed relationship with Derek is also the number one topic of conversation and therefore knows very well what it is like to be talked about, is committed to the patient's privacy. Alex looks after a young patient in a wheelchair and uses it to collect bonus points from Derek. Derek tries to forget about Meredith to save his marriage, and Addison wants to move to Seattle for good. Preston tells Richard about their relationship against Christina's will. Friends from New York visit Derek and Addison, but not without reason: Savvy tested positive for a gene for uterine and breast cancer that has already killed several of their relatives. She therefore wants a mastectomy and removal of her uterus and ovaries. Alex and Izzie get closer. George treats a suicide patient and Meredith an old woman who is diagnosed with cancer, but who wants to enjoy the last six months with her husband. | |||||||

It's thanksgiving. Izzie attaches great importance to the festivities, but Meredith has been assigned to the hospital. George is also prevented because he has to shoot a turkey with his brothers and his father. George realizes how little he has in common with his family. Meredith and Derek treat a coma patient who wakes up after 16 years and realizes that his family is no longer the same. Shortly after he wakes up, he has to undergo an operation and dies. Derek can't keep his appointment with Addison because of the surgery. In the evening the two decide to start all over again and kiss. Richard is off, but does not want to go home. Preston is also off. There is a substitute senior physician in the hospital who treats Miranda like a beginner and only wants to work with the "Nazi", a doctor with an excellent reputation - without knowing that this is Miranda's nickname. Preston goes to Izzie's dinner with Cristina. Joe also comes with his friend Walter. Preston helps Izzie with the cooking, which Cristina displeases. She withdraws to the hospital and assists in an operation. Alex reports to Meredith that he failed the medical exam. At the end of the day, Alex, Cristina and George come to see Izzie. Alex and Izzie have trouble in bed because Alex has erectile dysfunction. He speaks to his ex-girlfriend Olivia, whereupon they both have sex - and get caught by Izzie. In the hospital, the focus is on a woman with quintuplets who is facing a difficult birth. Labor begins too early and almost all of the hospital staff help deliver premature babies, which have various complications. | |||||||

After Izzie catches Alex red-handed with Olivia, she is mad at both of them. Izzie resents the fact that Meredith is still talking to Alex. Alex has to deal with the fact that he has a patient on his conscience because of a mistake. The quintuplets are cared for by the five interns. However, Izzie's baby dies and Izzie is grave. Addison explains to her that she knew beforehand that the baby would have no chance. Addison wanted to use this as a teaching method to teach Izzie more distance from patients, which Izzie cannot understand. An imprisoned murderer who has swallowed razor blades to escape the loneliness of solitary confinement is brought to the hospital. Your doctors will be Cristina and Miranda. When she is due to be discharged soon after her operation, she swallows a lightbulb. This ruptures the windpipe, which is why the patient has to be operated again. The nursing staff is on strike. George joins the strike. That's why he is laughed at by the doctors, but celebrated by the nurses like Olivia and Tyler. Through the strike they get Richard to work for more nursing staff. Meredith finds out that Richard is visiting her mother and that there is more than friendship between the two. When she sees an old woman die in the hospital with loved ones, she realizes that her mother needs much more affection. Meredith is comforted by Derek. Because of a young pregnant patient who comes from a background similar to Izzie, Izzie's old wounds break open. It is learned that Izzie became a mother when she was 16 and put her baby up for adoption. Because of Miranda's maternity leave, the interns are assigned a new doctor: Dr. Sydney Heron. Sydney is the complete opposite of Miranda, which Cristina - unlike Alex - doesn't get along with at all. | |||||||

Miranda comes back to the hospital as she is in labor. Her husband Tucker Jones has a serious car accident on the way to the hospital and has to be operated on by Derek. In addition, a patient is admitted with live ammunition from a bazooka in his chest . The ammunition is stabilized by the hand of the paramedic Hannah, who actually only reached into the patient's chest to stop the bleeding. All operating theaters are evacuated, only Derek refuses to interrupt the operation of Miranda's husband. When Hannah can no longer withstand the pressure and pulls her hand out of the patient's chest, Meredith sticks her hand in the wound and is now in mortal danger herself. The move succeeds, but only seconds after Meredith has removed the ammunition from the patient's body with the help of an explosives expert, the ammunition explodes. The explosives expert dies, Meredith gets away with minor injuries. In the evening she tells Derek that the only thing she could think about the whole time was that she couldn't remember her last kiss. Derek remembers and tells her about it. | |||||||

Preston finds out that Cristina kept her old apartment. Richard suspects that Meredith knows about his former affair with Ellis. Miranda visits her job with her child. Addison's former affair, Mark Sloan, shows up in Seattle to retake Addison and performs an operation on a patient. Denny Duquette is admitted again and Izzie only has eyes for him - to the chagrin of Alex. Addison contracted a poison oak rash while urinating in the woods . Meredith tries to apologize to George, but he doesn't accept her apology and doesn't want to see her. Cristina has to babysit for Miranda because Miranda has to lead an important operation and is overwhelmed by it. Alex is still jealous of Denny, with whom Izzie spends more and more time. On Izzie's advice, he ultimately decides in favor of an operation, which extends the time in which he can wait for a donor heart, but which will tie him to the bed in the hated hospital until then. Derek treats a patient with a brain aneurysm who does not want to have an operation at first because her husband has only paid attention to her since this diagnosis. Meredith has a date with vet Finn and they get closer, but Meredith wants to take things slowly at first. Derek is mad at Meredith after learning about them. George spent the night with Callie and then moves back into his old room in a shared apartment. Izzie can't stand Callie though. Preston's musical role model is operated by him, but he dies, which is why Preston blames himself. Miranda still suffers from not being assigned surgeries after she becomes a mother. Addison has Alex, who is not interested in gynecology , assigned as a resident. A mother of seven asks Addison to cut her ovaries without her religious husband knowing. Addison makes it look like a complication, but the husband sues her after Alex advises him to do so. Addison then tells Alex that he will have to continue working in her department as a punishment. Doctors have to look after an entire family who was involved in a car accident caused by an intern at Mercy West Hospital because of fatigue. The pregnant daughter dies as a result of the accident, but her baby survives due to the courageous efforts of Alex. George takes a step towards Meredith. Izzie and Meredith gossip about Callie, who makes it clear to George that he has to stand by her as her friend. Preston is mad at Cristina for falling asleep while having sex. Denny strains himself and has to go back to the hospital bed, which is difficult for him. Izzie tries to cheer him up, which is difficult for her. Many patients are admitted to the emergency room, some of whom were critically injured in a shooting in a restaurant. Preston makes his way to Mercy West with Alex, as a donor heart is ready for Denny. Complications arise and the heart becomes unusable. In order to get a different heart that was supposed to go to a patient who is on the organ donation waiting list before Denny, Izzie lies to Preston about Denny's condition. To get the necessary test results, Denny's condition actually has to get worse. So Izzie cuts the vital LVAD line and hopes that Preston will soon arrive with the donor heart. However, this is shot and dangerously wounded by the restaurant shooter in front of the hospital. Callie confesses her love to George, who does not return her words. Preston has a bullet stuck in his shoulder that partially paralyzes his right hand. Derek has to operate on him, but complications arise and Derek has to wake Preston up during the operation to test the functionality of his hand. Cristina is supposed to calm him down, but is completely overwhelmed by the situation. After Denny's donor heart has still not arrived, the four interns try to keep Denny alive for so long. Miranda finds out about this and suspends Izzie from duty. Just before Denny's surgery, which was replaced by Dr. Erica Hahn from Mercy West performs, he proposes marriage to Izzie. George apologizes to Meredith, knowing even better than she that Meredith doesn't love him. Richard's niece, whose ovarian cancer has returned, is admitted to the hospital. Meredith and Derek's dog has to be euthanized because of its cancer. After Persten's operation he left a tremor in his right hand, which he didn't tell Derek. The five interns stand by Izzie, who accepted Denny's marriage proposal, and give Richard no information about who cut Denny's tube. As a punishment, the interns are no longer allowed to operate and instead have to hold a ball at the hospital for Richard's niece, who has cancer. Derek and Meredith have sex there. Izzie also wants to go to the ball, but wants to visit Denny again before that. However, Denny has died in the meantime. Izzie is supported in her grief by her friends. Finally she admits her mistake to Richard and quits. |
