The men's 4 x 400 meter relay at the 2019 Pan American Games took place on August 10 at Villa Deportiva Nacional in the Peruvian capital, Lima .

32 runners from eight countries took part in the run. The gold medal was won by the relay from Colombia after 3: 01.41 min, silver went to the United States with 3: 01.72 min and the bronze medal was won by Trinidad and Tobago with 3: 02.25 min.
