The men's 20 km walk at the 1991 World Athletics Championships was held on the streets of Tokyo on August 24, 1991 .
In this competition, the goers from the Soviet Union won two medals, silver and bronze. World champion was the Italian defending champion, Olympic champion in 1980 , two-time Olympic bronze medalist ( 1984 / 1988 ) and Vice European Champion 1986 Maurizio Damilano . He won ahead of Mikhail Shchennikov . Bronze went to Jauhen Missjulja .
The two leading walkers Maurizio Damilano and Mikhail Shchennikov reached the stadium together. Shchennikov sprinted and crossed the alleged finish line first, but had to discover that there was still another lap to complete. In his attack, however, he had lost so much strength that it was no longer enough to win. Damilano finally won by three seconds.

Record improvements
- The Italian world champion Maurizio Damilano improved his own world championship record by 1:12 minutes to 1:19:37 h in the competition on August 24th .
- In addition, the Chinese Li Mingcai set a new Asian record with 1:21:15 h.
