The women's marathon at the 1991 World Athletics Championships was held on August 25, 1991 in the streets of the Japanese capital, Tokyo .

World champion was the Polish Wanda Panfil . She won ahead of the Japanese Sachiko Yamashita . Bronze went to the German Katrin Dörre , who had won the Olympic bronze three years earlier - at that time still starting for the GDR .
