After Indians had previously lived in the area that would later become Old Town, the first white settlers began to settle on the island in 1774. With the construction of the first sawmill in 1798, timber production, which was crucial for the town's economic development, began . From the middle of the 19th century, a rail link to the south of Bangor provided a connection to its port, which gave Old Town an advantage over other cities determined by the timber trade, as finished timber products could now also be transported. In addition, Old Town benefited from the fact that the Penobscot River was used by paddle steamers from 1848 .

The Interstate 95 runs in a north-south direction by Old Town. It runs parallel to the Stillwater River.
