The women's 4 x 400 meter relay at the 1991 World Athletics Championships was held on August 30 and September 1, 1991 in the Olympic Stadium in the Japanese capital, Tokyo .

The Soviet Union became world champions with the line-up of Tazzjana Ljadouskaja (final), Ljudmyla Dschyhalowa , Olga Nazarowa and Olha Wladykina-Bryshina (final) as well as Anna Tschuprina and Tatjana Alexejewa, who were also used in the run-up .
Second place went to the United States with Rochelle Stevens , Diane Dixon , Jearl Miles (final) and Lillie Leatherwood as well as Natasha Kaiser, who was also used in the run-up .
Bronze went to Germany in the line-up of Uta Rohländer , Katrin Krabbe (final), Christine Wachtel and Grit Breuer (final) as well as Annet Hesselbarth and Kathrin Schreiter, who were also used in the preliminary run .

The runners who were only used in the preliminary run also received the corresponding precious metal.

Record improvement

The world championship relay from the Soviet Union ( Tazzjana Ljadouskaja , Ljudmyla Dschyhalowa , Olga Nasarowa , Olha Wladykina-Bryshina ) improved the existing World Cup record in the final on September 1st by two tenths of a second to 3: 18.43 min.
