The 2011/12 Prva Makedonska Liga was the 20th season of the top football league in Macedonia . The season began on July 31, 2011 and ended on May 23, 2012 with the 33rd and final round.
FK Škendija 79 started the season as defending champions after the club won their first championship in 2010/11 . However, the title was not defended, the championship was won by record champions Vardar Skopje .

The two best teams in the second division rose to the Prva Liga. These were the FK 11 Oktomvri Prilep and the FK Ohrid , which both made their debut in the top division. The last placed FK Pelister Bitola had to relegate. Vardar Skopje was spared relegation as eleventh-placed team. This was made possible by a license swap in the course of a merger and re-establishment with the upgraded FK Miravci .
The ninth of the first division FK Napredok Kičevo won the relegation game against the third of the second division GFK Tikveš and remained in the top division. The tenth-placed FK Skopje , however, lost the relegation game against the fourth-placed second division club FK Miravci with 1: 4 and had to relegate after only one season in the top division. However, Miravci was not allowed to advance due to the reasons listed above.

Relegation
The ninth and tenth placed teams played in simple playoffs against the third and fourth placed teams of the second division for relegation. Both first division teams were able to win the games and therefore remained in the Prva Liga.
