Like over 100 meters , there was a duel between Jamaica and the United States. All three representatives of the two associations had qualified for the final, which was completed by a runner each from the Bahamas and the Cayman Islands.

The favorites were primarily the US double world champion from 2005 and 2007 Allyson Felix and runner-up Veronica Campbell-Brown from Jamaica. At the 2004 Olympic Games , however, the order was reversed. The four other runners from the USA and Jamaica were candidates for the other medals. For the United States, Muna Lee, who was third in her preliminary run here in Beijing and fifth over 100 meters, and Marshevet Hooker, who came second in her semi-final run, ran. Kerron Stewart and Sherone Simpson from Jamaica had won silver together in the 100 meter final four days earlier. The 2007 World Cup third Susanthika Jayasinghe from Sri Lanka was at the start, but could not reach the final here.

Even out of the curve, Campbell-Brown led quite clearly in the final. Felix and Stewart followed, Lee a little further behind. On the home stretch, Veronica Campbell-Brown kept her lead and repeated her Olympic victory from four years ago. With an excellent 21.74 s she was more than four tenths faster than in Athens in a slight pushing wind . It got tighter behind in the fight for the other medals. Allyson Felix prevailed against her competitors and was third nineteen hundredths of a second behind Campbell-Brown. It was even closer to the next seats. Muna Lee showed very good stamina and got closer and closer to the runners in front of her. But it wasn't quite enough for a medal. Sherone Simpson saved her third place seven hundredths of a second behind Felix and only one hundredth of a second ahead of Muna Lee. Marshevet Hooker was fifth ahead of Sherone Simpson. So there was no triple victory for Jamaica like over a hundred meters. As the best runner who did not come from the USA or Jamaica, Debbie Ferguson-McKenzie from the Bahamas took seventh place.
