Prémilhat is about 73 kilometers northwest of Clermont-Ferrand . Prémilhat is surrounded by the neighboring communities of Domérat in the north, Montluçon in the northeast, Lavault-Sainte-Anne in the east, Lignerolles in the southeast, Teillet-Argenty in the south and Quinssaines in the west.

Route nationale 145 runs along the north-western edge of the municipality .
