McNairy County was formed on October 8, 1823 from parts of Hardin County. It was named after John McNairy , a US District Court judge for the Tennessee District.
Three buildings in the county are listed on the National Register of Historic Places (NRHP) (as of August 23, 2018).

According to the 2010 census , McNairy County had 26,075 people in 10,113 households. The population density was 18 people per square kilometer. Statistically, 2.53 people lived in each of the 10,113 households.
The racial the population was composed of 92.1 percent white, 6.1 percent African American, 0.3 percent Native American, 0.2 percent Asian and other ethnic groups; 1.2 percent were descended from two or more races. Hispanic or Latino of any race was 1.6 percent of the population.
