Friedrich Stelzner (born November 4, 1921 in Oberlohma , West Bohemia ; † June 5, 2020 in Bonn ) was a German surgeon and university professor . He dealt primarily with visceral surgery as well as with functional anatomy and its importance for operations .

Life

Stelzner was the only child of the railway chief inspector Georg Stelzner (1889–1959) and his wife Helene Brandner (1908–1969). The mother came from Eger , where her father was a master shoemaker.

education

Friedrich Stelzner spent the first years of his life in Franzensbad . In nearby Eger he attended the city high school. After leaving school in the summer of 1939, Stelzner signed on as officer candidates for the medical service of the armed forces . Immediately after graduating from high school, he was drafted into the army (Wehrmacht) . The following year he was given permission to study medicine. Interrupted by assignments as a medical field doctor on the Eastern Front, Stelzner studied medicine from 1940 to 1945 at the Friedrich-Wilhelms-Universität zu Berlin , the Julius-Maximilians-Universität Würzburg , the Hessian Ludwigs-Universität and the Ludwig-Maximilians-Universität München . In Berlin he passed the state examination. With a doctorate in Curt Elze he was summa cum laude for Dr. med. PhD. After the end of the war, Stelzner began working as a surgical assistant doctor with Otto Goetze at the Erlangen University Hospital . In 1949 he became a specialist in surgery. Three years later he completed his habilitation on the radical removal of rectal cancer while preserving anal continence.

1954–1967: Senior physician in Hamburg

On the advice of his mentor Goetze, Stelzner moved to Hamburg as a senior physician in 1955. He was awarded a British Council Fellowship for additional training in colorectal surgery at St. Mark's Hospital in London. The cooperation with the leading visceral surgeons shaped him decisively. Inspired by this experience, Stelzner wrote the first edition of his standard work on anorectal fistulas. For this, Stelzner was awarded the Langenbeck Prize in 1960. With the Hamburg anatomist Jochen Staubesand (1921–2012) he worked out the exact vascular structure of the hemorrhoids. He found that hemorrhoids are not dilated veins but arteriovenous cavernous cushions. These pillows are very important for the anal continence. He also established the term anorectal continence organ and pointed out that this organ has significant differences in men and women. These structural differences are of great importance in anorectal surgery. Further studies uncovered the spiral structures of the esophageal muscles. Stelzner worked with the anatomist Werner Lierse on the basis of fluoroscopy specimens and serial sections of esophagi that were processed in situ . Knowledge of the true course of the wall muscle fibers of the esophagus forms the basis for surgical procedures for the treatment of achalasia and gastroesophageal reflux disease today .

1967–1987: Full Professor of Surgery

In 1967, Stelzner was offered a chair in surgery at the Eberhard Karls University in Tübingen . At about the same time, the Hamburg chair became vacant, and in 1968 Stelzner became a full professor at the University Medical Center Hamburg-Eppendorf . Two years after taking over the Hamburg clinic, Stelzner accepted a chair in surgery at the University of Frankfurt . There he worked closely with the professor of comparative anatomy Dietrich Starck , with whom he described the envelope fascia of the rectum and neck as important key structures for the oncologically correct removal of malignancies . Further investigations (again with Werner Lierse ) described the morphological basis of appendicitis and diverticulosis . In 1976 Stelzner received simultaneous offers to take over surgical chairs in Vienna and Bonn . After lengthy negotiations, Stelzner decided on the full professorship in Bonn. A large number of morphological studies from this period deal with the causes of the pilonidal sinus and pyodermia sinificans as well as with the embryology and the function of the pelvic fascia. In 1985 Stelzner became President of the German Society for Surgery .

From 1987: retirement and active retirement

Stelzner's retirement took place in 1987, but he headed the university clinic for another two years until his successor took office in 1989. In the years that followed, Stelzner regularly carried out minor surgical interventions. From 1995 Stelzner was no longer active as a clinician. From then on, however, his main focus was on scientific work. A major contribution to innovation was Stelzner's application of PET-CT to display the spontaneous activity of gastrointestinal sphincter systems . Other work established the existence of two separate lymphatic systems , located below the epithelia and deep within the mesenchyme .
