The road leads from Baia Mare (Frauenbach), where it branches off from Drum național 1C (also European route 58 ) to the east and the Drum național 18B coming from the south joins, via Baia Sprie and the 987  m high Pasul Gutâi and Vadu Izei pass to the The town of Sighetu Marmației (Marmaroschsiget) on the Tisza and the border with Ukraine , where it takes up the drum național 19 . There is a border crossing to Ukraine. The DN18 continues up the river Viseu via Vişeu de Sus , branches off the Drum național 17C to the south on the western edge of Moisei , passes through Borşa , where the short Drum național 18A branches off , and finally reaches its highest at the Prislop pass 1416  m Point. From there, it descends to Carlibaba and finally reached the village Mestecanis in Iacobeni (Suceava) north of Vatra Dornei the Drum naţional 17 (European route E58) on which it ends.

The length of the road around the Prislop Pass, described as being in poor condition, is around 220 kilometers.
