Acting: Peter Blake & David Shore
After House disbanded his team on the final episode of Season 3 and he hasn't taken on any cases in the past two weeks, Cuddy asks him to conduct interviews. He should choose doctors who work in his team. House is against this request as he thinks he can solve any case on his own. He wants to prove this claim in the case of treatment of Megan Bradberry, a patient who was buried alive and seriously injured after a house collapse. During the course of the episode, however, symptoms and clinical pictures emerge that the patient should not actually have according to the patient's file. The case is resolved by House by discovering that the actual patient is not Megan, but her colleague Liz and thus the mixed up medical files were the problem of the treatment. However, Cuddy refutes his claim that he doesn't need a team by stating that any member of his former team would have solved this case faster.

For this reason, she asks House to put together a new team, regardless of the means. As you can see in the last shot, House is addressing this demand by letting 40 candidates run. Gregory House's very special approach will make the following episodes rather turbulent, because the application process that begins has little to do with conventional methods.

House now faces a problem bordering medical science and occult phenomena or superstition. Of course, he doesn't believe in the cat's psychic abilities, and his team very quickly determines that the patient is “ Münchhausen ”, despite green urine. But then the woman gets worse and worse and shows severe symptoms that she could not have caused by manipulation. House also tests Kutner, who seems to be quite superstitious to him.

Main diagnosis: cancer in the appendix

The other patient, played by Mos Def , appears to have locked-in syndrome ; his thoughts are trapped in his body and he cannot talk to the doctors. A long first part of this episode is only told from the inner perspective of the injured cyclist. House eventually takes him on as a patient and begins investigating the cause in Princeton-Plainsboro . The end result: the traffic accident was caused by the illness identified, as the cyclist recognized the danger but could not brake. He was infected by a rat at a friend's house.

Main diagnosis: The locked-in syndrome was caused by leptospirosis .

106 20th The biggest step Simple explanation Apr 6, 2009 Nov 9, 2009 Greg Yaitanes Leonard Dick
The episode begins in the Novacks' house. Eddie is dying when his wife Charlotte stops breathing. Soon, however, the team's differential diagnosis is replaced by the absence of Dr. Overshadowed Kutners. Thirteen and Foreman find him at home, shot. Obviously it's suicide . The team is particularly shocked because no one noticed any signs of depression or anything like it. House begins a criminal investigation into the death. His research annoys the bereaved and remains without result: There is probably only one "simple explanation".
Meat Loaf plays the role of Eddie Novack.

A guest in a restaurant stands out for his outrageous behavior. He has "no more control over his left hand," as he knows himself. (Medically, it is the alien hand syndrome in split brain , see below.)
Gregory House can be satisfied. Since his boss Lisa Cuddy saved him and stayed with him overnight, Amber's threatening hallucinations are over. His pain is gone even without Vicodin. But it's too good to be true. Because the experiences that House likes to remember are not real.

In addition, a patient from the outpatient area appears, an elderly gentleman who sometimes suffers from a croaking voice and who follows House into his office; House thinks heartburn is the cause. The end is near when House does the same thing as the young patient - he offends the most important woman of his life - only that the patient doesn't do it willingly. House, on the other hand, loudly announces in the large lobby of the PPTH that he has slept with Cuddy, his boss. Then there is the fact that House has to realize that he had mistakenly assigned a suspected cancer to the wrong patient.

Ashton Holmes plays the young man with the uncontrollable hand, Carl Reiner the older man, and the music plays while Cameron and Chase get married. The final sequence shows the happy wedding celebration of the two doctors and - in parallel montage - House on the long, gloomy path to a distant place: Wilson drives him to the gate of the psychiatric clinic “Mayfield”.

Main diagnosis: Arrhythmia induced by propylene glycol with increased alien hand syndrome
Secondary diagnosis: Clear suspicion of pancreatic cancer (in the elderly man)

In the framework story, House lies on the upper floor of a burning house next to a corpse and hallucinates conversations with dead companions (Kutner, Volakis). In flashbacks, House treats a drug addict, forcing him to rethink his own life. The corpse next to him is that patient. Flashbacks show House trying to get his prison sentence extended through Foreman and Wilson, unsuccessfully. Since this would mean that he would not be allowed to see the last few months that Wilson still had to live, he decided to commit suicide. The hallucinated conversations eventually lead House to reconsider his decision. He gets up and wants to get out of the house. Through various clues, Foreman and Wilson find out where House is, reach the burning house and see an outline approaching the door when an explosion shatters the house. Based on the dental file, the body found in the house is identified as House. During the memorial service everyone in attendance finds good words about House, except for Wilson, who calls him a "monster" and a "bitter sadist". During this tirade, Wilson receives a message on his cell phone: “ Shut up, you idiot ” (German: “ Shut up , idiot”). Wilson actually runs into House, who escaped through the back door and thus survived the fire. He swapped the dental files to be officially dead and to be able to spend the remaining months with Wilson. Wilson reproaches House about the fact that he will never be able to work as a doctor again and that he will never be able to resume his life, but is visibly pleased. In the final scene, Wilson and House are out and about on motorcycles.

Main diagnosis: autoimmune disease due to inhalation of a piece of plant material
