Saint-Médard-de-Guizières is located about 46 kilometers northeast of Bordeaux and about 20 kilometers northeast of Libourne on the river Isle . Saint-Médard-de-Guizières is surrounded by the neighboring communities of Coutras in the north, Camps-sur-l'Isle in the northeast, Saint-Sauveur-de-Puynormand in the southeast, Petit-Palais-et-Cornemps in the south, Lussac in the southwest as well Abzac to the west.

The A89 autoroute runs through the municipality .
