L'Hôpital-le-Grand is located about 22 kilometers north-northwest of Saint-Étienne on the Forez . L'Hôpital-le-Grand is surrounded by the neighboring communities Boisset-lès-Montrond in the north, Unias in the east and northeast, Craintilleux in the east and southeast, Sury-le-Comtal in the south, Précieux in the west and Grézieux-le-Fromental in the Northwest.
