The commune lies on the banks of the Thérain river , into which the left tributary Petit Thérain flows, and on the railway line from Beauvais to Le Tréport ; the branch line to Formerie , which largely followed the course of the Thérain, has been dismantled. The former Route nationale 1 runs through the east of the municipality . The districts of Milly-sur-Thérain are Campdeville, Courroy, Moimont, Les Forges, Bury, Les Cardonnettes and Beaupré.
