draw
The draws for the games in the qualification and group stages took place on December 20, 2017 at the CONMEBOL Convention Center in Luque (Paraguay) . The teams were seeded in the Copa Libertadores according to their CONMEBOL ranking. Three factors were taken into account:
- Performance over the past 10 years, taking into account Copa Libertadores results for the period 2008-2017
- Historical coefficient, taking into account the Copa Libertadores results for the period 1960–2007
- National championship with bonus points to national champions of the last 10 years
The first and second qualifying rounds and the group stage were drawn. No draw was made for the third round of qualification, the pairings were determined in advance. The numbers in brackets are the result of the CONMEBOL ranking.

mode
If there was a tie in the group stage, the goal difference was decisive for progress, then the number of goals scored, then the number of goals scored away . If these are also the same, the lot decided. In the knockout rounds, the away goal rule also applied in the event of a tie in points and goals. If their number is the same, a penalty shoot-out immediately followed without extra time .

Round of 16
The first legs took place between August 7 and 21 and the second legs between August 28 and 30, 2018.
A few hours before the second leg between CA Independiente and FC Santos was due to take place on August 28 , the result from the first leg (0-0) was revised by CONMEBOL. Independiente had appealed against the rating of the first leg. These argued that the player Carlos Sánchez was used irregularly by Santos. He got a red card in 2015 when he was still playing for River Plate . Because of this, according to Independiente, he was still banned for the first leg. The association agreed to this objection, on the grounds that a three-year ban had not yet expired. The first leg was then rated 3-0 for Independiente. Curiously, the association announced three hours before kick-off for the second leg that Carlos Sánchez was entitled to play in this. The referee ended the second leg prematurely nine minutes before the end. After mutual provocations on the part of the fans, riots broke out and they tried to get onto the field.
Qualification for the quarter-finals will be rewarded by the COMMEBOL with an amount of 900,000 US dollars.

Quarter finals
The first legs took place from September 18 to 20 and the second legs from October 2 to 4, 2018.
In the first leg between Boca Juniors and Cruzeiro Belo Horizonte , Cruzeiro's center-back Dedé was sent off by the referee in the 76th minute after a clash with Junio's goalkeeper Esteban Andrada . In the scene of the game, both players went to the ball almost at the same time and met each other rather unhappily. Neither player claimed a foul. During the treatment break , Andrada suffered a broken jaw , but the referee watched the scene in the video evidence and then pronounced the dismissal. After a lot of support from the media, even from Argentina, Cruzeiro and the CBF, the national association of Brazil, protested against blocking the player for the second leg. The Conmebol held this protest on September 27, 2018. Dedé was allowed to participate in the second leg, in which Dedé received the yellow-red card in the 81st minute.

Return leg
The second leg was originally scheduled for November 24th. On the way to the stadium, River Plate fans attacked and damaged the Boca Juniors team bus. Several Boca Juniors players were injured. As a result, the game was initially postponed for one day and then canceled.
The second leg should now take place on December 8th or 9th of that year, but outside of Buenos Aires and Argentina. This was decided by the CONMEBOL association after the riots. The Italian city of Genoa has shown interest with the Stadio Luigi Ferraris . The Brazilian Belo Horizonte also wanted to host the game with the Mineirão . On November 29th, CONMEBOL confirmed that the final second leg will be played on December 9th in the Spanish capital Madrid at the Estadio Santiago Bernabéu .
Both clubs' stadiums remained closed to public viewing events. The traditional place for the victory celebration at the Obelisk in Buenos Aires, on the other hand, was cleared for the fan rush of the victorious club.
Due to a suspension from the semi-finals, Marcelo Gallardo , the coach of River Plate, could not look after the team in the second leg on the bench. The task was performed by his assistant Matías Biscay .
