When the conclave met on March 31, 1721, it counted 56 participants, the number of which has since fallen to 54 after the Cardinals Paracciani and Salerni had to leave the conclave due to illness; Cardinal Paracciani died on May 9, 1721, one day after the election of Innocent XIII. At the end of the conclave, however, 56 participants were again present, as the Cardinals d'Alsace and von Schönborn joined them on the evening of May 7, 1721 . Only ten cardinals stayed away from the election. The cardinals da Cunha and de Lacerda met only after the election of Innocent XIII. in Rome. The venue of the conclave was the Vatican.

Cardinal Mihály Frigyes Althan declared the veto of Emperor Charles VI. against an election by Cardinal Fabrizio Paolucci . The King of Spain vetoed the election of Cardinal Francesco Pignatelli seniore , OTheat , Archbishop of Naples and Cardinal Bishop of Sabina. Cardinal Michelangelo dei Conti was unanimously elected Pope and took the Pope's name Innocent XIII. on. He was the last Pope to date to choose this name.
